Guide for upgrading from 4.2 to 5.0 (packaged)

There used to be a guide for packaged upgrade from version 4.2 to version 5. I remember that there were several manual steps that were required to successfully move from 4.x to 5. I can’t seem to locate this guide and the current upgrade guide assumes you are already on version 5 and just updating to a later 5.x release. Can anyone help me locate the guide for upgrading from 4.2 to 5. Thanks
