[bug][backlogs] Task board, new task dialog without OK and Cancel

Bitnami OpenProject 4.0.4-1.
Browser Chrome, updated.

Backlog plugin, taskborad of a sprint.
If I press the “+” button of an user story, everything works well.

Click on the ID in the upper right corner of the yellow box of the user story to display its information, then close it.
Now, when I press again the “+” button to add a new task the small dialog appears without the Ok and Cancel buttons.

Workaround: press F5 on the browser to reload the task board page.

I hope you appreciate my little effort on highlighting the various issue and bug. As a developer (embedded word), I do like to have the best feedback, to keep higher the quality of products.
