It will also be very helpful if we could link project activities (WBS) to potential issues or risks and also link that again to an activity. that way, one can not only record an issue or a risk, but relate the issue or risk to the project plan (if it applies to a specific WBS) but then also make sure that the issue or risk is not only recorded but auctioned and that is what the linking of the issue with an activity will solve.
Adding notes or comments on issues, assigning the responsibility of an issue to e person or team will also help to draw people’s attention to it.
Would love to get feedback on this and if it can be done, how long before this can be done etc?
The reason for this statement/question, I do not want to ask a question or raise a need and leave it there never to become a reality.

An approach is to create a version called “issues” (found in the project setup option)
As you get new issues, create them as stories in the “issues” version.
You can assign the story to someone, you can create tasks (even better) and assign them, you can also relate these issues (workpackages) to your project tasks (WBSE’s)
you can create a couple more status’s to help manage them like “mitigated” or “ignored” :) if you like although, to keep thing simple “new, ”in progress" and “closed” should be ok.
you can report on them, give them due dates…
